The STAT Medical Info Mate (MIM) is a USB device that contains an exclusive medical questionnaire that you can fill out at your leisure, on your home or office computer. This simple questionnaire allows you the opportunity to fill in all the important medical information, no matter how extensive, that is needed to protect yourself in case of emergency. After completing the questionnaire, simply remove the USB device from the computer and store it safely on a keyring or in your wallet.
For routine office visits or procedures, simply give the MIM to your health care provider so they can access your medical history, current medications, dosages, and all of the important information needed to provide you with the appropriate care.
In the event of an emergency, the Star of Life symbol on the MIM will alert Medical Professionals to check the MIM for important Medical Information when you need it. All important questions needed by health care professionals are listed on this questionnaire.
